On an Average Day You Will:

(includes but not limited to)

• Provide skilled nursing care under the direction of an RN or physician
• Follow and verify the Plan of Care at each visit
• Communicate changes in condition to the RN Case Manager and physician
• Document care, medications, and patient progress accurately
• Educate patients and families on self care and safety
• Help prevent avoidable hospitalizations and ER visits

What You Bring:

• Graduate of an accredited nursing program
• Active Indiana LPN license
• At least one year of clinical experience
• Home health or hospice experience preferred
• Valid driver’s license, insurance, and reliable transportation
• Ability to lift, push, pull, or support up to 50 pounds
